Care in the Community

The Eleventh Hour Group community team is a service available 24 hours a day to support service users in any community setting throughout England and Wales. We specialise in providing support on a last-minute basis and then can continue with a consistent team of staff for as long as needed. Eleventh Hour Group supports local councils, social services, hospital discharges, domiciliary care, private clients, and companies.

All community services supplied by us are overseen by our Clinical lead who is a registered nurse with a breadth of experience in all areas of community provision.

Services for Adults

Individuals with learning disabilities can be supported in their own homes, supported living accommodation and holiday breaks. Eleventh Hour Group staff are specially trained and experienced to support individuals with these needs. Always looking to include the individual in all decision making as much as possible and promote independence. Support can range from full care requirements to support with accessing community activities.

Individuals with a condition that causes them to have physical disabilities, for example MD, MS, PD, ABI can be supported in their own homes, and on holiday breaks. Depending on the needs of the individual, Eleventh Hour Group would provide specially trained and experienced registered nurses or carers to support such individuals with their needs. This support can be 24 hours a day or other requirements involving full care or to support accessing community activities. Promoting independence where possible is always the main aim, as well as ensuring a person’s dignity is maintained.

Individuals requiring support due to their mental health can be supported by qualified nurses and/or support workers who are specialists in that field. Nurses can support with therapeutic interventions if required, and work with support workers to enable individuals to remain safe whilst promoting independence. Eleventh Hour Group is an advocate for reducing restraint, whilst all staff are trained in this area, they predominantly use all other approaches.

Palliative Care – Eleventh Hour Group can provide nurses and carers to offer essential care and support at the end of life in an individual’s own home. All staff are trained in this area and work with families and other services to ensure the individual is treated with dignity and compassion.

We can provide carers as required to support existing domiciliary care providers who may have short term or long-term staffing shortfall that requires intervention. All carers are experienced in this area of work and would be able to drive and quickly integrate into any system that is being used, and be a productive member of the team from day one.

District Nursing - Eleventh Hour Group can provide nurses and support workers as required to support district nursing teams throughout the country, who may have a short or long-term staffing shortfall. All of our staff are experienced in this area and would be able to drive and quickly integrate into any system that is being used.

Services for Children

Children with learning disabilities can be supported in their own homes supporting parents or guardians, as well as when additional support might be needed, for example for a holiday. Eleventh Hour Group also provides support staff to registered children’s homes for long or short-term requirements. Our staff are specially trained and experienced to support children with these needs. This support ranges from full care requirements to support with accessing community activities.

Children requiring support due to their mental health or other conditions, for example Autistic Spectrum Disorder, can be supported by qualified nurses and/or support workers who are specialists in that field. Nurses can support with therapeutic interventions if required, and work with other professions and support workers to enable children to remain safe whilst promoting independence. Eleventh Hour Group is an advocate for reducing restraint, and although it is necessary for all staff to be trained in restraint practices, other approaches are used unless specifically authorised.

Families requiring additional support due to ‘Child in Need’ or ‘Child Protection’ orders, can be assisted by support workers to help children remain in their own home safely. Daily reporting is provided by staff to ensure that professionals involved are kept abreast of all situations. All staff are experienced and trained in supporting children and aware of safeguarding.

Mother and baby support to new mothers who may be requiring assistance as identified by social services. Staff can be placed in a hospital setting to support and then continue in the family home.
